Boarding Contracts/Liability

Where would I be able to find a boarding contract for NYS? I need one to bring the Dutch WB mare to the farm in 2 weeks because the farmer's wary of any liability should she get hurt (especially since she's in foal) during the lease. My trainer also thinks it's a good idea, but she's on vacation so I can't pick up the one from the other farm to model it off of. Thanks so much!

Something like this, but I'm not sure how to tweak it regarding leases of mares while they're pregnant and boarded.
