Noble causeway to have surgery

According to bloodhorse he was scoped after the preakness and they discovered an entrapped epiglottis.

Could this have happened in the Derby when he threw his head up or would it have been discovered it by then?

It could have happened in the race. And lesser problems with "swallowing the tongue" sometimes aren't seen without a scope during treadmill exercise".
